URGENT: 8th Grade Park Day Permission Slip

Dear Parent or Guardian,

Eighth grade students will be taking a field trip to Humphrey Park on Tuesday, May 20, 2025 from 9:00 AM - 2:00 PM. Cafeteria services will prepare sack lunches only for students who request a school meal by returning this slip by Thursday, May 8th. Students may bring a sack lunch from home if they choose.

Students are ineligible to attend this event if they have received more than 3 days of ISS during 4th Quarter, 1 or more days of OSS during 4th Quarter, if they are on a safety plan, or if they have not completed their MAP testing.

If your child is eligible and you want your child to participate, please sign the form and return it by Thursday, May 8th. Otherwise, the cafeteria staff will not have necessary time to prepare the appropriate number of lunches.

On the day of the event, please send your students with sunglasses, or a cap, sunscreen, and a drink. Each student will be provided with a bottle of water. They are allowed to bring their own sports equipment, but we will not be responsible for anything lost, damaged, or stolen.

To be clear, the permission slip does need signed in order to attend, even if they don't need a school lunch. Thank you!!
